In your opinion ...why is health insurance so expensive?

Please , I'd like to hear some ideas of how and why our country can't provide excellent health insurance for a nominal price. Other countries do it,why can't the US? Are you perplexed as to why the non-working have benefits and the working don't without paying through the roof?
